During the verse of this song it is just screwing around on C#m.  The 
bass just plays a pedal on a 6-string bass on C# (I wouldn't know that 
without the farewell video)  The only part with actual chords is the 
chorus's, they are the following:

C#dim:	9	10	8	8	x	x
G/(C#)*	9	10	12	(12)	x	x
F#/C#	9	9	11	(11)	x	x

(*the C# is really there with the orchestra but sometimes it doesn't 
sound right on the guitar)

also in some of the verses the orchestra has a little progression using 
the following:
	E	A	D	G	B	e
C#m:	x	4	6	6	5	x
G/C#:	x	4	5	4	x	x
F#/C#:	x	4	4	3	x	x
A:	x	0	2	2	x	x
although these are the same chords as the verse I personally like the way 
they sound in this position.  Use them as follows:
